On November 14, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of lack of supervision and lack of an accident or injury report. This complaint is substantiated for the lack of an accident/injury report. Parent #1 did not receive a phone call, email, or message regarding the missing hair braid. Administrator #1 stated an injury report was not written because Parent #1 is always upset when the facility calls her. Staff #2 stated she attempted to talk to Parent #1 about the missing braid but Parent #1 walked past her to the classroom. Staff #1 acknowledged an incident report should have been written and stated Parent #1 was not contacted because Staff #1 became busy. Staff are required to notify parents of any accidents or injuries before the child leaves for the day. There is no evidence to substantiate for lack of supervision. According to the camera footage, Staff #1 was monitoring Child #1, and all staff interviewed provided similar accounts of the events leading to Child #1 missing a braid.
