OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On May 9, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline. This incident was self-reported to OCCL and to the Child Abuse and Neglect Report Line by the facility on May 8, 2024. Staff #1 was terminated from employment on May 7, 2024. On July 12, 2024, OCCL received information Staff #1 would be substantiated at Level 2 physical abuse on the Child Protection Registry. On August 27, 2024, OCCL received information Staff #1 had been arraigned and the case closed with adult arrest. Based on the documentation received, this complaint is substantiated for improper discipline.

An investigation was begun on September 6, 2023, and concluded on September 20, 2023. On October 7, 2022, OCCL received a complaint regarding a lack of supervision. An investigation was conducted on October 10, 2022. Based on the results of this investigation this complaint is substantiated because LS Flowers and Budin observed video footage of Staff #1 using her laptop for lesson planning while Child #1 had a paper in his hands and mouth for several minutes.
