OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. Between February 19, 2025, and February 20, 2025, three complaints were reported to the Office of Child Care Licensing (OCCL) regarding concerns of improper discipline. On February 18, 2025, Administrator #1 self-reported the incident to Licensing Specialist #1. An investigation was conducted on February 26, 2025. Based on the results of the investigation, this complaint is substantiated for improper discipline, as video footage reviewed by LS #1 and #2 showed Staff #2 grabbing Child #1 by one hand and arm and lifting them into the air on two separate occasions. Following an internal investigation, Staff #1 and Staff #2 received disciplinary write-ups placed in their files and are required to complete refresher training on the program's practices for promoting children s healthy social and emotional development (Pyramid Model). Additionally, both staff members will receive individual coaching sessions from the program s Classroom Quality Assurance Coach. Additional concerns were identified during the investigation, including the use of cell phones in the classroom and failure to maintain proper classroom ratios. Video footage showed Staff #1 using their cell phone multiple times to text and answer a phone call while in the classroom. Additionally, Staff #2 was observed leaving the classroom with the lunch cart for approximately four minutes, despite attendance records indicating that thirteen children were signed in and present during lunchtime
