The Office of Child Care Licensing (OCCL) received a complaint on May 26, 2026, regarding concerns of operating over licensed capacity, not using positive behavior supports, children walking to and from the bus without permission/lack of supervision, not following proper diapering procedures, insufficient toys being available and toys being in disrepair, lack of cleanliness and sanitation, and rodent infestation. An investigation was conducted beginning May 27, 2026, and ending on June 2, 2026. This complaint is substantiated for operating over licensed capacity based on observations on June 2, 2026. Nine children were present, with only one school-age child in care. This complaint is substantiated for not using positive behavior supports; while all persons interviewed denied having children hold their hands in the air as a disciplinary measure, Provider #1 noted there s not much re-direction with the children as they are toddlers, and all persons interviewed noted time-out or standing in a corner as a regular means of discipline. This complaint is substantiated for not following proper diaper changing procedures based on observations and interviews. Substitute #1 stated they have changed a child on their lap and observations of a diaper change showed that sanitation and handwashing procedures were not followed, and the diaper was not disposed of in a foot-activated trash can. This complaint is substantiated for lack of cleanliness and sanitation. Although Provider #1 noted all materials are cleaned and sanitized as required, LS #1 and Licensing Supervisor #1 observed two heavily stained pack-and-plays in disrepair. There is no evidence to substantiate this complaint for children walking to and from school bus based on observations and interviews conducted. Provider #1 and Substitute #2 stated children are not unsupervised when walking to and from the school bus. LS #1 and Licensing Supervisor #1 observed children walking to the bus stop and were accompan ied during each on site visit. There is no evidence to substantiate this complaint for insufficient toys in child care spaces based on observations. The family child care home has sufficient toys for DELACARE Regulation requirements for the children present at the time of the visit. There is no evidence to substantiate for rodent infestation based on interviews conducted. Provider #1 stated there have been no rodents inside the family child care home. Multiple additional concerns were noted during the visit. Adult #1 was unsupervised and transporting school-age children to school without a completed comprehensive background check and without written approval for this arrangement from any of the parents of children in care. No attendance records were available for children or substitutes during both visits during this investigation. Multiple children's files were either unavailable or incomplete. The posted fire drill log had not been updated since February 2026. Multiple hazards were accessible to children during this investigation. In the kitchen, cleaning chemicals were on the floor, a broken drawer was sitting at child level with knives in it, a cabinet was broken, and there was injectable medication unsecured in the refrigerator. Upon arrival at the home, a diffuser was running in the child care space. A foot operated trash can in the bathroom was observed with a soiled diaper and no liner. Substitute #1 was noted to be regularly used in non-emergency situations and was present at both of these visits without being approved through OCCL as a non-emergency substitute. An updated infant feeding schedule was not available for Child #1; the last one completed was from February 2026.

The Office of Child Care Licensing (OCCL) received a complaint on May 20, 2024, regarding concerns of the provider not living in the child care home, the vehicle used for transportation not having proper registration, and insufficient food being provided to the children. An investigation was conducted on May 28, 2024. There is no evidence to substantiate this complaint for the provider not living in the child care based on observation of the upstairs of the home, the interview conducted, and verification of the address on Ms. Jackson s driver s license. The Office of Child Care Licensing (OCCL) received a complaint on July 7, 2023, regarding concerns of operating while suspended and improper discipline. An unannounced suspension follow-up visit was conducted on July 7, 2023, and an unannounced visit to investigate this complaint was conducted on July 17, 2023. There is no evidence to substantiate this complaint for operating while suspended based on two visits with no children present within the home. Ms. Jackson denies providing care since her license was suspended on June 23, 2023. There is no evidence to substantiate for improper discipline as no children were observed in care.

On June 23, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding the family child care home being overcapacity, meals and snacks not having all required components, lack of materials, and the licensee not being present as required. An investigation was conducted on June 23, 2023. This complaint is substantiated for the family child care being overcapacity, meals and snacks not having all the required components, and lack of materials. Upon the licensing specialist s arrival, 16 children were present in the family child care home, which has a capacity of nine children. The menu observed did not contain all the required components for meals and snacks. The licensing specialist did not observe enough materials available for all the children to use. There was no evidence to substantiate the allegation that the licensee is not present and providing child care at all times except during the limited use of a substitute. Two of the people serving as substitutes stated it is not typical for the licensee to not be present at the family child care home. However, the information provided does not support what the licensing specialist has observed since October 2022. The licensee was not present when the licensing specialist arrived for the last three unannounced facility visits. Additional concerns noted were the licensee did not notify OCCL that she was using three additional people as substitutes; one of the people serving as a substitute has not been fingerprinted for the family child care home; there was no attendance documentation for the three people serving as substitutes; there was no smoke alarm present on the child care level of the home; and 12 children present did not have files.

The Office of Child Care Licensing (OCCL) received a complaint on January 19, 2023, regarding a minor being responsible for child care duties, requirements regarding the use of a substitute not being followed, lack of supervision, the licensee not notifying OCCL of a change in substitute, and exceeding licensed capacity. An investigation was begun on January 20, 2023, and concluded on February 9, 2023. This complaint is substantiated for lack of supervision, not following requirements regarding the use of a substitute, not notifying OCCL of a change in substitute, and for exceeding capacity, based on observations at the time of the visit and information provided by a representative of Purchase of Care (POC). There is no evidence to substantiate the complaint for a minor being responsible for child care duties because both people present were over the age of 18. Additional concerns noted during the investigation were: two persons were present at the family child care home caring for children in the absence of the provider who had not been fingerprinted for the facility or received an eligibility or provisional eligibility notification, two children were observed sleeping on the couch, one infant was sleeping in an infant rocking chair, and four children were on cots with no bottom cover; two children who were present did not have the required child files.

On July 19, 2021, the Office of Child Care Licensing received a complaint regarding concerns about Child #1 having a red mark on her finger and stating a staff member hit her. At the conclusion of an investigation conducted on July 27, 2021, there is no evidence to substantiate this complaint. No marks were visible on Child #1, and the provider and substitute deny hitting Child #1 or disciplining Child #1 for anything on the day of the allegation.
