On September 23, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding diapering, accident/injury procedures, not providing truthful information, and not supervising children on the playground. An investigation was conducted on September 25, 2025. This complaint is substantiated for accident/injury procedures because while reviewing the accident/injury reports, two entries which occurred more than a business day prior, did not have a parent/guardian signature. There is no evidence to substantiate the complaint for diapering noncompliance because staff interviewed stated they complete diaper checks every hour and change diapers when soiled. There is no evidence to substantiate the complaint for not providing truthful information because, although the reporter alleges Child #1 hit his/her head when a play car tipped over, all staff interviewed stated Child #1 did not hit their head and incurred no injury from the incident. There is no evidence to substantiate this complaint for lack of supervision because staff interviewed stated they were actively supervising children on the playground. Additionally, both Staff #4 and Administrator #1 stated they watched video footage which showed Staff #1 and Staff #2 supervising the children appropriately and stated Child #1 did not hit their head.

On July 10, 2025, and July 14, 2025, the Office of Child Care Licensing (OCCL) received complaints regarding not having an understanding of and respect for children and their needs, a lack of positive behavior supports, and not providing physical care routines appropriate to each child s developmental needs. An investigation was conducted on July 16, 2025. This complaint is substantiated for not providing physical care routines appropriate to each child s developmental needs because, according to video footage reviewed, Staff #1 was not able to effectively provide for the toileting needs of Child #1, and asked for assistance from other children in the classroom. There is no evidence to substantiate the allegation children were continually discussing Child #1 s nudity throughout the day, because all staff interviewed stated the children did not talk about it. A concern noted was Staff #1 was observed on video entering the school-age bathroom, closing the door, and remaining in there several minutes, leaving the classroom unsupervised. Through interviews, it was also stated a child was sent, unsupervised, from the school-age classroom to the front office. Another concern noted was when asked about Staff #2 stimulating pressure points on children s bodies, the video was reviewed and while it showed Staff #2 doing this, it is unclear whether the children were in pain or harmed. A concern noted while reviewing the video footage was Staff #2 is seen and heard yelling very loudly at two separate children. According to the staff interviewed, the yelling was heard in the office. Staff #1 s employment was terminated on July 11, 2025, and Staff #2 s employment was terminated on July 7, 2025.

On March 24, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding diapering procedures. An investigation was conducted on March 27, 2025. There is no evidence to substantiate this complaint because according to staff interviewed, diaper checks are completed hourly, and diaper changes are completed as needed. Documentation also shows the times of those checks and changes according to video footage reviewed by staff for a specific day. A concern noted is while checking for Child #1 s diaper cream and Medication Administration Record (MAR), it was determined several children did not have a completed MAR for the diaper creams being used in the classroom.

On October 22, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding proper diapering procedures and interactions. An investigation was conducted on November 6, 2024. There is no evidence to substantiate this complaint because all staff interviewed stated Child #1 utilizes the toilet when prompted during the day, eliminating the need to change his diaper. Classroom staff, Staff #2 and Staff #3, stated they treat his diaper like a pull-up and continue to re-use it since it is not wet or soiled. Additionally, Staff #2 stated they encouraged the parent to bring in pull-ups for him. Regarding appropriate interactions, both Staff #2 and Staff #3 stated Child #1 is responsive to their directions and instructions, he plays with other children, and behaves typically for a three-year-old.

On April 18, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding a spray being used on the children s nap mats. An investigation was conducted on April 25, 2024. This complaint is substantiated because Staff #3 stated she used a lavender scented air freshener in the doorway area of her classroom, as well as on the children s nap mats prior to nap time. Non-compliance Cited: Reg #49: Ensure air fresheners are not used in any child care space. Staff #3 stated she used a lavender scented air freshener near the door and on the children s nap mats prior to nap time. Administrator Renee Christian and Staff #4 removed the air freshener from the classroom and completed a written warning for the staff. Additionally, all the other classrooms were checked for air fresheners.

On March 19, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, not providing truthful information to a parent, and not notifying OCCL when follow-up medical or dental treatment is needed after an accident or injury which occurred at the center. An investigation was conducted on March 26, 2024, and March 28, 2024. There is no evidence to substantiate this complaint for lack of supervision because Staff #2 stated she watched video footage which shows Staff #5 following Child #1 into the bathroom only a few seconds after she went in. Regarding the allegation of not providing truthful information to a parent, Staff #1 stated she saw cracked teeth after the injury on March 15, 2024, but thought Child #1 s teeth were always like that. Staff #4 stated she looked in Child #1 s mouth after the injury and did not see any broken teeth. According to electronic communications viewed, Owner Danielle Harrison asked for notification if Child #1 received any follow-up medical treatment. However, according to Staff #2, the center was never notified Child #1 went to the dentist. A concern noted is when questioned how long Staff #1 has worked with Child #1, she stated she has been in the same classroom with Child #1 for over a year; however, she did not notice Child #1 s broken teeth after the injury.
Disposition: No Evidence to Substantiate
A complaint was received by the Child Abuse and Neglect Report Line on January 30, 2024, and accepted for investigation by the Institutional Abuse Unit (IAU). The complaint was sent to OCCL and accepted for investigation regarding improper discipline. On February 6, 2024, IAU stated the case would be abridged because after reviewing the video footage, the incident does not rise to the level of abuse because no injuries were sustained by the children involved. OCCL conducted an investigation February 7, 2024. The complaint is substantiated for improper discipline because video evidence shows Staff #1 roughly handling children, including pulling them by one arm, pushing them, picking them up and dropping them onto the floor, and frequently using time-out without any explanations to the child. Additionally, two other staff in the classroom, Staff #3 and Staff #4 did not intervene to protect the safety of the children in care. Staff #1 was immediately terminated from employment prior to the start of this investigation. Staff #3 walked out during her shift and has not returned; therefore, resigning without formal notice. Staff #4 met with Ms. Christian and received a final written warning with corrective action. On February 21, 2024, Owner Danielle Harrison notified OCCL she terminated Staff #4's employment due to this incident.

The Office of Child Care Licensing received a complaint on November 21, 2023, regarding not meeting the needs of children by allowing feces to remain on a child s bottom, having staff complete trainings for other staff, having a plumbing issue and allowing children to crawl in the dirty water, and the center being dirty and having roaches. An investigation was conducted on November 21, 2023. There is no evidence to substantiate this complaint because Staff #1 denies knowing any time when a child has left the center with feces on his bottom. All staff interviewed stated they are allowed to complete training hours at the center when the classrooms are in ratio, and no one has offered to complete trainings for them. Interviews and documentation reviewed show the plumbing issue was resolved in a timely manner and parents were notified accordingly. The center did not have any significant cleanliness issues and a pest control company was utilized immediately to remediate the roach that was seen in the maintenance closet.

The Office of Child Care Licensing (OCCL) received a complaint on May 12, 2023, regarding not following the requirements for pets. An investigation began on May 16, 2023, and ended on May 18, 2023. This complaint is substantiated because Staff #2 stated she was not sure she informed every parent of the pet rats in the two-year-old classroom.

The Office of Child Care Licensing (OCCL) received a complaint on March 27, 2023, regarding the center not following its termination policy and staff not following diapering procedures. An investigation was conducted on March 29, 2023. Based on interviews with staff and documentation provided, there is no evidence to substantiate the allegations. Staff stated they check children s diapers every hour, change diapers every two hours and as needed, and check each child s diaper before they leave for the day. Documentation showed staff were following proper diapering procedures. Additionally, the owner terminated care for a child due to the child s parents behavior toward the staff, which aligns with the termination policy in the parent handbook.

OCCL received a complaint on June 11, 2022, regarding lack of supervision and improper positive behavior support. An investigation was conducted on June 13, 2022. The complaint is substantiated for lack of supervision because staff admitted Child #1 was left unattended on the playground for approximately 4 minutes. There is no evidence to substantiate this complaint for improper positive behavior support because staff denied the allegation.
