The Office of Child Care Licensing received three complaints regarding unqualified staff alone with children, improper staff-to-child ratio, staff not having CPR/First Aid, lack of security procedures, and meal pattern requirements. The investigation began on January 13, 2026, and concluded on January 23, 2026. This complaint is substantiated for unqualified staff alone with children, improper staff-to-child ratio, staff not having CPR/First Aid, and lack of security procedures based on observations and documentation reviewed. There is no evidence to substantiate the allegation of not following meal pattern requirements because during the visit, staff and children were eating the meal on the menu with the proper serving size, and leftovers were available. An additional concern noted was based on attendance records there were not enough staff present in the building in the morning.

The Office of Child Care Licensing received a complaint on January 16, 2026, regarding a lack of positive behavior supports. An investigation was conducted on January 21, 2026. This complaint is substantiated because staff and children stated that Staff #1 and Staff #3 have told children when they have asked for support to figure it out themselves rather than providing appropriate adult guidance, supervision, or intervention.

The Office of Child Care Licensing received a complaint on December 3, 2025, regarding improper staff-to-child ratio and lack of supervision. The investigation began on December 10, 2025, and concluded on December 16, 2025. The allegation of improper staff-to-child ratio is substantiated because upon arrival, there were 18 school-age children with one staff member. Also, according to attendance records, the facility did not follow proper staff-to-child ratio on December 2, 2025, from about 6:30 am until 8:00 am. The allegation of a lack of supervision is substantiated because during the visit the specialist observed on two occasions that a school-aged child moved around the facility alone, once to use the bathroom and once to enter another classroom. Interview statements indicated that staff members are on their phones, leaving the classroom, in offices, or not engaged with children in the school-age classroom when needed for supervision. Other concerns noted were a lack of security procedures to limit access to children while in care, unqualified staff alone with children, not having enough staff present in the building, and not having staff trained in day-to-day operations present.

On October 29, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding release of a child and termination policy. An investigation was conducted on October 30, 2024. This complaint is substantiated for non-release of a child because Parent #1 was on the child information card, and the child was not released to Parent #1. Owner #1 was being cautious in not releasing Child #1 because Owner #1 believed a court order existed for no contact but was not aware of specifics in the order; Owner #1 contacted law enforcement for assistance in resolving the issue. However, without the presence of the actual court order informing Owner #1 of the specific terms of the no-contact order and confirming Child #1 was involved in that order, Child #1 should have been released to previously authorized individuals. The facility did follow regulation requirements by not allowing Person #1 to retrieve Child #1 due to not being an authorized pick-up. There is no evidence to substantiate not following the termination policy because the handbook states the center reserves the right to terminate care for any reason.

On April 11, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding a lack of supervision. An investigation was conducted on April 11, 2024. The complaint is substantiated because Staff #4 stated she did leave the classroom, leaving children with no supervision, to get a child a cup of water from the kitchen. Additional concerns noted during the visit were the classrooms did not have water accessible to the children, the one-year-old to two-year-old classroom was over ratio with eight children and one staff from 8:50 a.m. until naptime, and the infant attendance record was not accurate. Non-compliance Cited: Reg #27 Ensure children are directly supervised at all times. Ensure staff-to-child ratios are maintained at all times. Reg #36: Keep a daily written or electronic record of each child s exact arrival and departures in each classroom or area. Reg #70 Ensure drinking water is always available to children in their classrooms and outdoors and supplied to them on their request or available for self-service as appropriate.

The Office of Child Care Licensing (OCCL) received a complaint on November 2, 2022, regarding accident/injury procedures, lack of supervision, insect infestation, staff using cell phones, not being truthful when providing information to parents, and positive behavior supports. An investigation was conducted on November 9, 2022. The complaint is substantiated because upon arrival, this specialist observed an aide without criminal history determination, alone with seven children; the youngest was one year old. There is no evidence to substantiate for not following accident/ injury procedures and staff using cell phones because all staff denied these allegations and accident/ injury reports were provided for all head injuries discussed with staff. There is no evidence to substantiate for staff not being truthful because information provided by Staff #1 and the accident/ injury report match. There is no evidence to substantiate an insect infestation because LS Sommer did not observe evidence of infestation and staff stated the issue was taken care of by an exterminator. There is no evidence to substantiate for positive behavior supports because all interviewed staff stated they do not have concerns regarding improper discipline or bullying at the facility. Additional concerns noted were insufficient light in two classrooms during naptime, and trash cans without enclosed lids were observed.

OCCL received a complaint on June 6, 2022, regarding lack of supervision. An investigation was conducted June 9, 2022. This complaint is substantiated for lack of supervision because according to all staff interviewed, Child #1 was able to leave the three-year-old classroom and come to the front office.

OCCL received a complaint on March 7, 2022, regarding release of children and not having enough staff to fulfill at a minimum, administrative, fiscal, clerical, cleaning, maintenance, food services, direct child care, and supervisory functions. An investigation was conducted March 15, 2022. This complaint is substantiated for not having enough staff to fulfill administrative duties because LS Sommer observed and Staff #1 stated the voicemails had not been checked since March 3, 2022, because administrative staff are needed in the classrooms. This complaint is substantiated for failure to release of a child because according to attendance, Child #1 was not released until 4:45 p.m. which supports the Reporter s allegation of being unable to pick up Child #1 earlier in the day when she attempted to pick Child #1 up.
