Ensure the interior space designated for children s use is available to children when the center is in operation. The South American classroom needs to be set for care. Ensure the use of air fresheners is prohibited. All classrooms and the common areas have automatic air fresheners.
Ensure fences are in a safe condition, self-close and self-lock, each outdoor space has 75 square feet for each child in the area, and that fall zones are the proper depth and at least 6 feet from the equipment. The fencing has sharp edges and pinch points. The small play area needs to be measured, and the slides and climbers in the area need proper fall zones.
Ensure that changes to the daily meal are documented. The breakfast menu listed cornflakes, a banana, and milk. The food served was KIX, Cheerios, or Life cereal, Fruit Punch, Juicy Juice, and milk.

Ensure that a plan review is submitted when making changes to the outdoor area. A file review indicated that the facility has three-year-old equipment and three outdoor play areas. The original plans only had one outdoor play area on file. No plan review was submitted for the replacement of equipment or fencing off areas.
Ensure that a comprehensive background check is completed every 5 years. The owner has not been printed for the center since 7-26-23.
Ensure staff to child ratio is maintained at all times. During te visit upon arrival there were 23 cgukdreb ub tge Australia classroom after the 90 minute of operation regulation. Specialists arrived at 9:15 am ninety minutes shold have benn 6:30 am - 8:00 am. This was corrected when staff took three two year old to the Europe classroom. Twenty children were in the Asia classrom with one qualified staff memger and one unqualified staff member. This was corrected when a qualified staff member joined the room.

An employee was allowed to work at the center prior to becoming eligible. Beginning January 1, 2023 a licensee shall ensure a staff member is fingerprinted and the licensee has received an eligibility determination or a provisional eligibility notification before the start of employment.
The center does not have enough staff to give breaks and or lunch, Ensure to hire staff so the center is able to run smoothly and ratios are able to be met.

Ensure the assignment of eligible, qualified staff members are physically present and working with children. LS McPhatter observed that staff #4 was alone caring for children and has out of state pending checks making them not currently eligible and is not qualified at minimum as an intern.

LS McPhatter did not have access to files and documents even though the day to day staff was present.

On the written documentation available staff #4 was not signed in with the 8 documented children.
Ensure a system for documenting children s attendance that includes the exact arrival and departure times in the child s classroom and be entered at the time of the arrival and departure of the child. LS McPhatter observed that the Europe classroom was not able to produce the electronic tracking due to internet connection issues. On the written documentation available 8 children were documented and 10 were present in the room.
Ensure the center is free of unacceptable exposure to hazardous materials. It was observed that the lower cabinets of the Europe and Australia classrooms are accessible to children and were unlocked.
Ensure all areas of the center in a clean and safe condition. Finishes must be cleanable and in good repair. The Australia classroom has broken furniture in the middle of the floor and a cubby shelf is broken. Ensure all unused electrical outlets accessible to children including unused power strips are covered. The indoor play area needs unused outlets covered. A licensee shall ensure garbage and trash are stored securely in covered containers. The Europe room needs a lid on the trash can.
A licensee shall ensure a kitchen or food preparation area is provided with the necessary operable equipment to prepare, store, serve, and clean-up all meals and snacks for children and staff. In the Europe classroom LS McPhatter observed dishes in a sink and dishes on a dish rack. Staff #4 confirmed that she washes the dishes and utensils in the classroom. The sink is a single compartment sink. Washing dishes in the classroom also impleads direct supervision regulation.
Ensure liquid soap, toilet paper, and paper towels or a mechanical hand dryer are available at all times in the toilet rooms. Ensure used disposable diapers are placed in a foot-activated trash can that is used only for diapers. This trash can must be within arm s reach of the changing area, lined with a plastic bag, and sanitized daily. Ensure a center has a diaper-changing area with a clean, washable, and non-porous surface, disposable covers must be used for each diaper change. The South America classroom needs a foot activated trash can in the bathroom for pull ups, a changing mat, and disposable covers. The Europe classroom needs soap, a covered foot activated trash can and paper towels in the bathroom. It was observed that the trash can and paper towels are kept in the classroom approximately 5ft from the sink in the bathroom.
Ensure the grounds are maintained in a clean and safe condition. The play areas have a green outdoor carpet that is blowing up or bunching and causing tripping hazards. Secure the carpet to the ground or remove it. Broken child size patio chairs are broken and need to be removed. There are compressed wood furniture parts on the play ground that need to be disposed of.

A licensee shall ensure vehicles used to transport children, including parent/guardian vehicles used for field trips (unless only transporting his or her own children), have and use the following: A working heater and air conditioner, a working phone, a traveling first aid kit, including children's emergency contact information, and a dry chemical fire extinguisher approved by Underwriters Laboratory. Use of a vehicle, other than a school bus, with a rated capacity as defined by the manufacturer, to carry more than 10 passengers in addition to the driver to transport children is prohibited. A licensee shall ensure 12-15 passenger vans are not used to transport children. Passenger includes the driver. It was disclosed to LS McPhatter that Ms. Ruff was transporting children in her personal vehicle until the tags come in for her new van. Ms. Ruff did not have the current transporting vehicle available to ensure the required items and documentation was in the vehicle. LSS Vandewater asked to see the new van and upon inspection of the van it was noted it is a 15 passenger van and Ms. Ruff was informed to not use the vehicle for transportation due to it not meeting current DELACARE regulations.
Ensure a child's rest equipment is labeled with the child's name or assigned chart number and used only by that child while attending the program. Ensure chart numbers and assignments are documented and kept current. The Europe classroom has a cot chart that staff #4 did not know the location of and the cots are not labeled with names or numbers to identify assignment.
