Ensure the daily record includes documentation that each infant was checked every 30 minutes when placed in the crib, playpen, or pack-and-play to nap or sleep. Staff members stated they observed each infant for normal breathing, signs of distress

On October 16, 2025, the Office of Child Care Licensing received a complaint regarding lack of supervision and accident/injury reporting procedures. An investigation was conducted on October 22, 2025. This complaint is substantiated for lack of supervision because although staff denied the child was injured while in care, during the visit, a group of children was left alone in a classroom while the staff answered the door. There is no evidence to substantiate the allegation of not following accident/injury reporting procedures because two classroom staff stated Child #1 had no injuries while in care. In addition, Child #1 was signed out at 5:05 p.m. and according to staff and Brightwheel documentation, the parent did not communicate Child #1 had an injury until 13 minutes after Child #1 left the facility. Additional concerns noted during the visit included improper staff-to-child ratio and insufficient staff onsite.

On March 24, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline and staff not following requirements regarding indoor space, diapering, sanitation, and lesson plans. An investigation was conducted on March 27, 2023. The complaint is substantiated for improper diapering procedures because the licensing specialist observed a staff member not wash a child s hands after a diaper change. The complaint is substantiated for staff not following indoor space requirements because 10 children were present in a classroom that had a documented capacity of six children. There is no evidence to substantiate the complaint for improper discipline because all interviewed staff denied the allegations. There is no evidence to substantiate the complaint for staff not following sanitation requirements because interviewed staff denied the allegation and stated they follow proper time frames for cleaning and disinfecting items, such as mouthed toys, soft toys, and tables. Toys in the classrooms appeared clean. Additionally, each classroom and bathroom had labeled cleaning and disinfecting solutions. There is no evidence to substantiate the complaint for staff not following lesson plans because the licensing specialist observed children doing activities documented on the lesson plans. Items posted in classrooms also matched activities on previous lesson plans. Concerns noted were an unqualified staff member was alone with three one-year-old children, and there were not enough staff physically present to fulfill the minimum daily functions, such as staff s breaks.
