The Office of Child Care Licensing (OCCL) received a complaint on June 22, 2026, regarding safe sleep. An investigation was conducted on June 29, 2026. This complaint is substantiated for safe sleep non-compliance because during the investigation there were four infants asleep in either a chair or boppy pillow, one staff member had a child asleep in their arms, and one infant had a blanket draped across them.

On May 27, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding improper staff-to-child ratio and lack of positive behavior supports. An investigation was conducted on June 4, 2026. This complaint is substantiated for improper staff-to-child ratio because according to the attendance records, on May 27, 2026, the Blue Room was over ratio and over maximum group size with two staff members and up to 23 children, the youngest being three, from 8:25 am until 8:35 am. From 8:35 am until 9:00 am, there were 12 three-year-olds with one staff member. There is no evidence to substantiate lack of positive behavior supports because staff stated they never withheld food from the child; the child refused to come to the table to eat.

On January 30, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding not having sufficient staffing, improper staff-to-child ratio/mixing toddlers with older children, the administrator not being present, diaper changing procedures, child attendance, and caring for children in the lobby. An investigation was conducted on February 4, 2025. This complaint is substantiated for child attendance because according to attendance records, one child was not signed in and two children were not signed out of the classroom on January 30, 2025. This complaint is substantiated for caring for children in the lobby because according to staff, they sat at the front desk with children in care and on January 30, 2025, attendance records showed a staff member had two children at the front desk. There is no evidence to substantiate for diaper changing procedures because staff stated children are changed before combining classrooms, and staff stated if children need to be changed after that, they will take them into their assigned classroom to change them. A concern noted was according to the attendance records, there were four staff not signed out of the classrooms. There is no evidence to substantiate for insufficient staff or improper staff-to-child ratio/mixing toddlers with older children because according to attendance records and staffing roster, there was enough staff present, all classrooms were in proper staff-to-child ratio, and the toddlers did not mix with the older children. There is no evidence to substantiate the administrator not being present because according to the staff roster, the administrator and teachers trained in day-to-day are present daily.

Ensure walls and floors are in good repair. Infant room has exposed screws, holes and peeling paint on the walls. Red room has dirt build-up on the floor and dirt on the wall by the sink. Yellow room has peeling tape on the floor collecting dirt. Ensure air fresheners and plugins are not in use. Infant room and Green room both had air fresheners. Ensure measured useable space for the capacity is maintained at all times. The Orange room was measured for capacity, but is currently used for storage.

Ensure cleaners, other than soap and water, are kept out of reach of children. Specialists noted cleaners in reach in the Blue, Green and Red rooms. This was discussed and corrected at visit when staff moved the items out of reach.
Ensure floors, walls and other surfaces are clean and in good repair. The ceiling in the Blue room has a tile ready to fall and needs to be fixed.
Ensure food is labeled with contents and expiration dates. Specialists noted several bags of cereal not labeled.
Ensure bathrooms are clean. The bathroom in the Blue room needs to be cleaned around the base of the toilet and under the floor mats.
Ensure staff files are complete. Staff #1 needs training hours, and PD plan. Staff #2 needs CPR/First Aid, annual training hours, PD Plan, health appraisal with TB and service letters. Staff #3 needs First Aid training and service letters. Staff #4 needs 1 hour of health and safety training. Staff #5 needs service letters. Staff #6 needs annual training hours. Ensure ratios are being maintained at al times. The specialist observed an attendance record for 11/2/22 where there 15 children present with 2 teachers from 9:20am-3:00pm, with the youngest child being one. This was discussed with the administrator, and the specialist observed classrooms in ratio. Ensure infants/toddlers are not being mixed with pre-school children outside of the allowed times. The specialist observed attendance records for 10/31/22 showing 2, 3, and 4 year old's being mixed in one classroom for the majority of the day. This was discussed with the administrator and she stated they have hired more staff and opened up more classrooms.

The Office of Child Care Licensing (OCCL) received a complaint on January 4, 2023, regarding the center not following policies on positive behavior supports and not providing a quiet activity for children who do not sleep during naptime. An investigation was conducted on January 8, 2024. This complaint was substantiated for positive behavior supports because one staff stated two children did not receive their snack due to their behavior and another staff stated two children refused to wash their hands and were delayed in getting snack until their hands were washed, so by the time they received the snack, it was time to move on to the next activity, so they did not have enough time to eat.. There is no evidence to substantiate not providing a quiet activity at naptime because all interviewed staff stated children have nap sacks they receive after 30 minutes of rest.

The Office of Child Care Licensing (OCCL) received a complaint on July 31, 2023, regarding lack of supervision. An investigation was conducted on August 3, 2023. This complaint is substantiated for lack of supervision because a child walked out of a classroom, approximately 20 feet down the hallway, with another family unnoticed by staff. The administrator was present at the front desk, in the hallway area, outside of the classroom, but was unaware of the child leaving the classroom. An additional concern noted was according to attendance records, an aide was alone with children in a classroom on July 27, 2023.

The Office of Child Care Licensing (OCCL) received a complaint on November 16, 2022, regarding cell phone usage and improper staff-to-child ratios. An investigation was conducted on November 22, 2022. This complaint is substantiated for improper staff-to-child ratio because during the visit an unqualified staff was observed alone with three children and staff disclosed an unqualified staff was alone with children on picture day. There is no evidence to substantiate the allegation of staff using cell phones for personal use because all staff interviewed denied the allegation. The administrator stated staff members use their electronic devices to input information into Pro Care, which is the electronic system the center uses to document children s daily activities.

The Office of Child Care Licensing (OCCL) received a complaint on October 10, 2022, regarding diapering, staff-to-child ratios, unqualified people alone with children, and mixing of age groups outside of allowed times. An investigation was conducted on October 14, 2022. The complaint is substantiated regarding staff-to-child ratios and mixing of age groups outside of allowed times because several staff members confirmed this to be true, and LS Smith observed several attendance records confirming the allegations. There is no evidence to substantiate not following diapering procedures or unqualified people alone with children, because LS Smith observed all staff following the proper diapering procedures and all staff denied unqualified people supervising children alone. An additional concern noted was staff were not documenting their arrival and departure times accurately on attendance records.
