On May 9, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding incorrect staff-to-child ratios. An investigation was conducted on May 14, 2025. Based on interviews and a review of attendance records, this complaint is substantiated for incorrect staff-to-child ratios. At the time of the investigation visit a classroom was observed to not be in correct staff-to-child ratio. In addition, a review of attendance records showed the two-year-old classroom having an incorrect staff-to-child ratio on May 9, 2025. Additional concerns were noted during this investigation. LS #1 observed an infant sleeping in a swing while wrapped in a fleece blanket. A staff member was working at the center without having been determine eligible or provisionally eligible by the Criminal History Unit, and both staff and child attendance had not been documented accurately.

The Office of Child Care Licensing (OCCL) received a complaint on July 22, 2024, regarding concerns of incorrect staff-to-child ratio and failing to complete an incident report and notify parents of an incident in the required time frame. An investigation was conducted on July 26, 2024. There is no evidence to substantiate this complaint for concerns of incorrect staff-to-child ratio based on attendance and video footage observed. The attendance records and video footage showed there were eight school-age children present with one qualified staff member. There is no evidence to substantiate this complaint for failing to complete an incident report and notify parents in the required time frame based on interviews conducted and documentation collected. Staff interviewed stated parents were notified of Child #1 s behaviors and there was a written statement regarding Child #1 behaviors.

The Office of Child Care Licensing (OCCL) received a complaint on February 16, 2024, regarding concerns of improper discipline and not following the facility s suspension policy. An investigation was conducted on February 21, 2024. There is no evidence to substantiate this complaint for improper discipline based on staff interviews conducted. Staff members stated files fell on Child #1 s head when she was crawling on the floor in the office. There is no evidence to substantiate this complaint for a violation of the center s suspension policy based on interviews conducted and documentation. Staff stated Child #1 was suspended from the facility, and they followed the facility s suspension policy and incident reports viewed.
