On December 27, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding incorrect ratios between the hours of 3:30 pm 5:00 pm. Based on staff interviews, observations, and documentation received, this complaint is substantiated for exceeding maximum group size because at the time of the visit, the number and age of children in the multipurpose room exceeded the maximum group size. The center quickly updated and educated staff about the new schedule.

Have teachers in classrooms get new MAR's for all medicines/diaper creams.
Staff files require the following documents: Health Appraisal with TB screening: Staff #2 and Staff #3 Administration of Medication Training: Staff #1 OCCL Health and Safety Training (7 hours): Staff #2, Staff #4, Staff #5.
Radon testing needs to be completed by the license expiration. Ensure all soap and water and sanitizer bottles are labeled. (Soap bottle in Little Angels, soap bottle in Little Cherubs, need labeled in Little Blessings, soap and water in Pre-K1)
Outlet covers needed in Little Lambs (1 missing) and Little Saints (in bathroom).
Replace thermometer in Little Stars room and Preschool II. Lower temperature in Little Cherubs refrigerator- was 50 degrees.
Ensure there are sufficient materials available in all classrooms: Little Stars room needs more dramatic play, building materials, and manipulatives Little Lambs room needs paper for art supplies and sensory materials Little Saints classroom needs blocks, manipulatives/ problem solving materials, and art Little Blessings room needs building blocks, manipulatives, music, and dramatic play materials. Send pictures of the materials in the rooms.
Diapering station needs to be within 5 feet of handwashing sink- this needs to be re-arranged in the Little Blessing room- send a picture when completed.

Affix the ventilation fan to the ceiling in the Preschool III bathroom. Clean the edges around the floor in the Little Blessings classroom. Send pictures.
Clean the fabric on the yellow chair in the Saints classroom. Fix/tighten the legs on the table in dramatic play in this classroom. Send pictures when this is complete.
Evacuation route needs to be posted in one of the classrooms.
Staff files were missing the following: - Staff #1 needs CPR/First Aid. - Staff #2 needs CPR/First Aid and proof of one service letter attempt. - Staff #3 & 8 need proof of attempts to send service lettesr. - Staff #4 needs one more reference. - Staff #11 needs 2 references and one service letter attempt. - Staff #13 needs first aid training and a health appraisal.
Child files were missing the following: - Child #5, 11, 14, and 15 need update health appraisals and immunizations. - Child # 12 needs an updated health appraisal, immunizations, and a lead screening.
Clean under the diaper mat on the changing table in three of the classrooms. Add a diapering mat into the Saints bathroom while waiting on delivery of a full changing table. Send a picture when complete.
Send copies of document signed for a staff member trained in day-to-day operations.
Ensure all bottles in the infant room are labelled with the children's names. Send a picture when this is corrected.
Ensure staff files are complete. Medication administration is needed for four staff members.
Ensure staff attendance is properly documented. At the time of the visit, one staff member was not signed into the toddlers class and one staff member was not signed out of the classroom.
Label hook in the school-age classroom.
Obtain additional equipment for the toddlers classroom.
Ensure lesson plans contain all required elements. Toddler room plans need social/emotional component. Toddler room needs art, blocks, and math/ problem solving areas.
Ensure bottles are warmed in water that is no more than 120 degrees. Replace bottle warmer in the infant classroom or create a system for monitoring the temperature.
Early Childhood Administrator needs Infant/Toddler and School-Age specialized training.
Lock the cabinets in the Little Saints room.
