On March 12, 2026, the Office of Child Care Licensing received a complaint regarding allegations of providing an inaccurate injury report, failing to follow policy regarding parent complaints, and not following their procedures to address food allergies. After interviewing Administrator #1 and reviewing all documentation, there is no evidence to substantiate this complaint regarding the allegations. The facility had not received documentation of any allergies for Child #1, followed DELACARE regulations for providing an accident or injury report, and documentation showed Child #1 was fed appropriately during the day in question.

On February 16, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision and failing to use appropriate behavior supports. An investigation was begun on February 19, 2026, and concluded on February 26, 2026. Based on interviews conducted and documentation reviewed, there is no evidence to substantiate this complaint for lack of supervision. All staff members stated they have not witnessed staff leaving the classroom while children are present. LS #1 reviewed attendance records and found all classrooms to be in correct staff-to-child ratio. Based on interviews conducted there is no evidence to substantiate this complaint for not using proper behavior supports. All staff members stated they have not witnessed coworkers failing to use proper behavior supports with children. A concern noted during this investigation was no staff member documented as trained in day-to-day operations was present in the absence of the administrator during the February 19, 2026, visit.

On October 4, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, not following procedures for contacting parents, and not following the video sharing policy. An investigation was conducted on October 11, 2024. Based on interviews conducted and documentation reviewed, there is no evidence to substantiate this complaint for lack of supervision. Staff #2 and Staff #3 both stated they were in ratio and never have over twelve children in their classroom. Staff were present in the classroom, interacting with children but unable to prevent the hair pulling. Based on documentation and interviews, there is no evidence to substantiate this complaint for failing to notify a parent when the incident occurred. Staff #1 stated and provided documentation of contacting Parent #1 after providing first aid to Child #1. Based on interviews there is no evidence to substantiate this complaint for not sharing video with parents. Staff #1 stated the cameras at this facility are live-stream only. Staff #1 stated there is no video to share. Staff #1 stated this is a Celebree corporate policy.

On July 24, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of positive behavior support and diapering and soiled clothing. An investigation was conducted on July 26, 2024. Based on staff interviews this complaint is substantiated for diapering and soiled clothing. The center is responsible for providing extra clothing and changing children when they are found to be wet or soiled. Based on staff interviews, the center did not have extra clothing available for school-age children. Also, clothing must be changed when it is found to be wet or soiled. Child #1 was not changed out of their soiled clothing until a parent arrived. There is no evidence to substantiate this complaint for lack of positive behavior support because staff stated the other children were guided away from the bathroom area to give Child #1 privacy.

On June 13, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline, failing to notify parents of an incident/injury, failure to report suspected abuse or neglect to the Report Line, and unqualified staff alone with children. On June 14, 2024, this report was called into the Child Abuse and Neglect Report Line by OCCL staff for review by the Institutional Abuse Unit (IAU). The report was initially screened in by IAU, but on June 18, 2024, OCCL was notified IAU was abridging their investigation and would not be pursuing it further. An investigation by OCCL began on June 17, 2024, and concluded on June 24, 2024. Based on the results of this investigation there is no evidence to substantiate this complaint for improper discipline because all four staff members who were interviewed denied ever observing any staff member use improper discipline with children. Also, Person #1 refused to cooperate with this investigation. There is no evidence to substantiate parents weren t notified of an accident or injury because both the center administrator and Staff #3 stated they were unaware of any children with bruises. There is no evidence to substantiate that the center did not report suspected abuse or neglect because Staff #2 was unable to provide information on which children were hurt, how the children were hurt, or when the occurrences happened. There is no evidence to substantiate unqualified staff are left alone with children because Staff #1 is qualified as an Early Childhood Intern and may be alone with children.

On October 13, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding a lack of supervision, incorrect staff-to-child ratios, not providing an accident/injury report to a parent, and not notifying the parent of an injury to a child. Based on the results of this investigation this complaint is substantiated for not providing a parent with an accident/injury report because Staff #1 was unable to provide this specialist with a copy of an accident/injury report for Child #1. This complaint is substantiated for not notifying a parent of an injury to a child because Child #1 s parent was not notified when Child #1 was punched in the stomach. There is no evidence to substantiate a lack of supervision because Staff #2 stated she was supervising the room at the time of the incident, but was assisting other children when the incident occurred. There is no evidence to substantiate incorrect staff-to-child ratios because attendance records for August 2, 2023, show the school-age classroom was in correct staff-to-child ratio.

On August 28, 2023, the Office of Child Care Licensing received a complaint regarding staff not using respectful communication. An investigation was conducted on September 1, 2023. Based on the results of this investigation this complaint is substantiated because the center administrator and Staff #1 both stated Staff #2 did not use respectful communication with Staff #3.

On November 3, 2021, OCCL received a complaint regarding failure to immediately notify a parent of a serious injury. An investigation was conducted on November 3, 2021. Based on the results of this investigation, this complaint is substantiated because the center administrator stated Child #1 s parent was not immediately notified of the injury to Child #1 s face. Per DELACARE regulations, any impact to child s head is considered serious and must be reported immediately to a parent or guardian.

On August 13, 2021, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline. An investigation began on August 11, 2021, and concluded on September 28, 2021. There is no evidence to substantiate this complaint because all six staff members who were interviewed denied observing any staff member use improper discipline and none of the four children who were interviewed were able to provide any information regarding the allegations of this complaint. Also, LS Flowers observed children in the classrooms participating in structured activities. There is no evidence to substantiate that the staff did not intervene when a child was being slapped by another child because one staff was putting the child s lunch away and the child s parent entered the room immediately to yell and retrieve the child.
