On May 27, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline, not following diapering procedures, failing to follow food service policies, lack of cleanliness in indoor spaces, spoiled food, the administrator not being present the required number of hours, and improper staff communications. An investigation began on May 30, 2025, and concluded on June 6, 2025. Based on staff interviews, documentation, and the video footage viewed, there is no evidence to substantiate this complaint for improper discipline. LS #1 viewed video footage and observed appropriate redirection of children by staff and children calmly exploring different areas in the room. Based on interviews and observation during the visit, there is no evidence to substantiate this complaint for not following diapering procedures. LS #1 observed diapering that meets the DELACARE regulations. Based on staff interviews and observation at the visit, there is no evidence to substantiate this complaint for failing to follow food service policies. The food is kept in the kitchen, and no spoiled or expired food was observed. Based on interviews and observation during the visit, there is no evidence to substantiate this complaint for lack of cleanliness. The staff explained how they clean the rooms, and the classroom had proper equipment to clean and disinfect. The classroom was observed to be clean at the time of the visit. Based on interviews and a review of documentation during the visit, there is no evidence to substantiate this complaint for the administrator not being present the required number of hours. LS #1 reviewed the attendance for Administrator #1 and Staff #2 which confirmed the Administrator's attendance. Based on interviews and observations during the visit, there is no evidence to substantiate this complaint for improper staff communications. Staff interviewed did not express any concerns regarding communication with the administrative staff. Admin istrator #1 and Staff #2 shared their policy on communication.

On May 14, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of improper sanitization and physical environment. This complaint is unsubstantiated. There was no mold observed, and Administrator #1 stated all repairs were completed after the flood. It was mentioned the carpets were cleaned and shampooed immediately after the flood. Additionally, the carpets and ceiling appeared to be in good condition upon arrival.

On August 28, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline and safe sleep. An investigation was conducted on August 28, 2024. Based on staff interviews and videotape viewed, this complaint is substantiated for improper discipline. Staff #2 lifted Child #2 using one arm and quickly moved Child #2 to lie down in the crib on the infant s stomach. The center acted quickly to remove Staff #2 from the center, and then terminated employment for Staff #2. Based on interviews and a review of video footage, this complaint is also substantiated for a safe sleep violation. Staff #2 was observed placing an infant down in their crib on their stomach, not on their back. Staff #2 s employment at the center has been terminated.

On August 20, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline. An investigation was conducted on August 28, 2024. Based on interviews conducted, there is no evidence to substantiate this complaint for improper discipline. An additional concern noted at the visit was Staff #2 had been inactivated as a staff member at the facility because of information supplied by the center. However, Staff #2 had returned to work without having a new criminal background check completed and receiving a new eligibility letter.

On August 1, 2023, the Office of Child Care Licensing (OCCL) received four complaints regarding concerns of improper security procedures, not following the center s open-door policy, not providing water to the children, not having a posted menu, and not following proper procedures for soiled clothing. This complaint is substantiated for not following proper security procedures based on interviews with staff members. The door to the facility was temporarily unlocked due to work being completed in the building. This was corrected on August 14, 2023, when a new security system was installed. There is no evidence to substantiate water not being available to children because water was observed in each classroom. There is no evidence to substantiate soiled clothing procedures not being followed based on interviews with staff, as they stated soiled clothing is put in a sealed bag, labeled with the child s name, and sent home the same day. There is no evidence to substantiate inaccurate attendance records for staff and children not being kept accurately because all attendance records viewed reflected the correct number of children and staff, and lunch breaks were recorded. There is no evidence to substantiate not following the open-door policy, as all staff interviewed stated parents are permitted in the center and classrooms. There is no evidence to substantiate menus not being posted, as they were observed posted in every classroom.
