Reviewed CH 1 file and observed no documentation of lost child incident or any documentation information was shared with family. Identified CH1 was lost for approximately 20 minutes and District communication center was notified. identiifed incident occurred at approximately 9:12, facility called communication center at approximatley 9:30 to notifiy of lost child. Identified Mom was contacted by phone of incident at 1:40 p.m. and Father at 2:04 p.m. Correction: Maintain documentation of written record of serious accident, illness or injury with copy provided to the parents/guardians.
