Identified CH1 was lost for approximately 20 minutes and District communication center was notified. identiifed incident occurred at approximately 9:12, facility called communication center at approximatley 9:30 to notifiy of lost child. Identified Mom was contacted by phone of incident at 1:40 p.m. and Father at 2:04 p.m. Correction: Maintain documentation of written record of serious accident, illness or injury with copy provided to the parents/guardians.
