At the time of the visit, a Febreze air freshener spray, labeled "Keep out of reach of children," was observed on the toilet bowl, making it accessible to children. Correction: Immediately ensure that all items labeled "Keep out of reach of children" are stored in areas inaccessible to children. 2.312.J · Within thirty (30) calendar days of caring for children the primary provider and all individuals responsible for the collection, review, and maintenance of the child immunization records must complete the Colorado Department of Public Health and Environment (CDPHE) immunization course. This training must be renewed annually.

At the time of the visit, the provider’s file was reviewed, and it was found that the Immunization Records FAX 303-866-4453 Phone 303-866-5948 ________________________________________________ training had expired on 9/17/2024. Correction: Review file to ensure it contains a current Immunization Records training certificate. Submit a copy of the certificate along with a written statement of compliance by July 9, 2025.
