2.120.I · Every five (5) years, all child abuse and neglect inquiry background checks must be renewed by resubmitting an inquiry form and current fee to the Department for processing. An updated clearance letter or verification of the submission of the inquiry form must be obtained before five (5) years from the date reflected on the current clearance letter.

Reviewed two (2) staff files. One (1) staff file contained a TRAILS clearance letter dated 12/13/2019. The expired TRAILS clearance letter was the only TRAILS clearance letter available for review at the time of inspection. The staff member did have current CBI and FBI clearance letters. 2.229.C · Items labeled “keep out of reach of children” must be inaccessible to children.

Two (2) gallon-size containers of Purell sanitizer/disinfectant cleaner with the statement "Keep out of reach of children" on the label were observed under the sink in the cabinet with the blue doors near the teacher's desk. The cabinet did not have a locking device. The sink is used by children and the containers of cleaner were accessible to children. Correction: Items labeled “keep out of reach of children” must be inaccessible to children. Correct immediately. Corrected at the time of the inspection by the assistant teacher removing the two (2) containers of Purell from under the sink and placing them in an area that is inaccessible to children. No response is needed for this violation. Correct by 05/21/2024

7.702.72.C.1.a · All climbing equipment eighteen (18) inches or higher must have resilient surfacing of at least six (6) inches in the use zone surrounding the equipment.

Four and a half inches (4.5") depth of resilient material (wood chips) in the use zone of the grey slide, and four and a half inches (4.5") of resilient material (wood chips) in the use zone of the climbing wall. Correction: Ensure that the use zone for all equipment eighteen inches (18") or higher contains at least six inches (6") of resilient material. Correct immediately and document compliance to the Department in your response. Correct by 08/29/2023
