This training must be renewed annually and may count towards ongoing training requirements.

Reviewed (4) staff files and observed the Program Director, who has been employed longer than (30) days and is responsible for the collection, review and maintenance of child immunization records, to have a CDPHE immunization training certificate that expired on 5/26/2026. Correction: Ensure all staff who are responsible for the collection, review and maintenance of child immunization records have completed the CDPHE immunization training within (30) days of employment and annually thereafter.
