    In the bathroom located between the Vail classroom and Steamboat classroom, observed baby wipes labeled "keep out of reach of children" on the back of a toilet accessible to children. This is a direct violation of stipulation M on the probationary license. Probationary stipulation M states “Hazardous items, inlcuding items labeled "keep out of reach of children" must be inaccessible to children at all times, both indoors and outside. Choking hazards must be inaccessible to children under 3 years of age.” Correction: Immediately, items labeled "keep out of reach of children" must be inaccessible to children. Corrected at the time of the visit. A staff member moved the baby wipes to a high cabinet making them inaccessible to children. No response is required. Correct by 02/17/2026
