This training must be renewed annually and may count towards ongoing training requirements.

Observed three staff records and observed staff members Lizz. L. (DOH 8.2014) and Anna P. (DOH 9.5.2023) to be missing documentation of the required (CDPHE) immunization training. Identified that both staff ,members are responsible for collecting and maintaining immunization records. Correction: All staff memebrs respsonsbile for the collection and maintence of immunization records must complete the required immunization training annually.
