The preschool playground lacking 6" of resilient surfacing at the bottom of the slide, prior to where the rubber matting is placed. Also observed several areas where grass and weeds are growing up through the wood chips, within the 6' use zone of the climbing equipment that exceeds (18) inches in height. Correction: Ensure there is at least (6) inches of resilient surfacing within the use zone surrounding all climbing equipment that is (18) inches or higher. Correct immediately and document plan of compliance with written response to Specialist by 10/6/2025. Correct by 09/29/2025
