At time of visit, observed S hooks to be used for the swings which had gaps in them, posing a strangulation hazard to children. Also observed fence panel to be broken, posing a safety hazard to children. Correction: Immediately, 6/14/2024, indoor and outdoor equipment, materials and furnishings must be sturdy, safe, and free of hazards. Corrected at time of visit by the S hooks being mended and the broken fence repaired. No response needed. Correct by 06/14/2024
