2.121.D · Each owner and employee sixteen (16) years of age or older of a facility or agency shall complete the fingerprint process at an approved vendor. Payment of the fee for the criminal record check is the responsibility of the individual being checked or the facility or agency. The results of the criminal record check, the CBI and/or Federal Bureau of Investigation (FBI) response letters, must be maintained at the home, center, facility, or agency and must be available for review upon request by a Licensing Specialist.
Reviewed nine (9) staff files. One (1) staff file did not contain documentation of the FBI clearance letter for the staff member that was available for reveiw at the time of inspection. The file did contain TRAILS and CBI clearance letters. 2.230.C.1.a · All climbing equipment eighteen (18) inches or higher must have resilient surfacing of at least six (6) inches in the use zone surrounding the equipment.
The wood chip resilient material was frozen and unable to be measured. At the time of inspection three (3) children were observed playing on the climbing play structure and slide that are over eighteen (18) inches in height. Correction: All climbing equipment eighteen (18) inches or higher must have resilient surfacing of at least six (6) inches in the use zone surrounding the equipment. When the wood chips are frozen they are not considered to be resilient and children should be kept off of equipment that requires the resilient surfacing in the use zones. Multiple large empty plastic bags that are used to store nap materials, were located in an open basket located against the front wall of the "PreK" classroom. Empty plastic bags that are used for nap materials were also located on top of a child-size table in the "TK" classroom and were accessible to children. Correction: Loose plastic bags must be stored in areas inaccessible to children. Correct immediately. Corrected at the time of the inspection by the lead teachers for the PreK and TK classrooms removing the plastic bags from the child care area and making the plastic bags inaccessible to children.
