Observed an individual identified as a "parent volunteer" assisting the providers during the inspection. Learned through conversation with the provider that two other individuals who are parents or grandparents volunteer on occasion at the childcare center and have done so recently. Provider stated that the volunteers help her less than 14 days per year. The three individuals identified as volunteers were not signed in as a visitor at the child care home. No documentation of the time and date the individuals visited and or volunteered at the child care home was available at the time of inspection. Correction: Visits from all non-family members to the home must be on the sign in/out log, including the name, date, and arrival/departure times. Please ensure that all visitors and volunteers document their arrival and departure from the child care home. 7.707.83.K · Outdoor physical free play materials shall consist of at least four (4) age appropriate toys and equipment including, but not limited to, the following in good repair: push toys, riding toys, tossing toys, climbing equipment, balance boards, stationary swings, slides, balls, toss games, and sports equipment. These must beprovided daily except in extreme weather, such as rain, snow, or extreme temperatures when indoor physical play may be substituted.
At the time of inspection, the blue curved slide was not in good repair. The bottom two (2) sections of the blue curved slide were coming apart where they were supposed to be attached more securely. This is creating a gap in the bottom two sections of the slide which is a hazard. Correction: Outdoor physical free play materials shall consist of at least four (4) age appropriate toys and equipment including, but not limited to, the following in good repair: push toys, riding toys, tossing toys, climbing equipment, balance boards, stationary swings, slides, balls, toss games, and sports equipment. These must be provided daily except in extreme weather, such as rain, snow, or extreme temperatures when indoor physical play may be substituted. Please ensure that children are not permitted to use the blue curved slide until it is repaired and the bottom sections are secured properly.
