Identified through discussion with the Provider, the large structure is not used during child care hours, however the children do access the portable climbing equipment during child care hours. Observed no evidence of at least 6 inches of resilient surfacing (wood chips) within the use zone of the portable climbing equipment. Correction: Immediately ensure all climbing equipment 18 inches or higher has resilient surfacing of at least 6 inches in the use zone at all times.
