A bottle of hand sanitizer labeled "Keep out of reach of children was found on a cabinet that was also accessible to children. Correction: Immediately ensure that all items labeled “Keep out of reach of children” are stored in areas that are inaccessible to children. This was corrected at the time of the visit as the provider moved the sanitizer to a high basket and locked the cabinet, making the items inaccessible to children. Correct by 04/23/2025

This training must be renewed annually.
At the time of the visit, reviewed the providers file and found that the provider has an expired standard precautions training. The provider's standard precautions training certificate expired on 2/21/2024. Correction: Immediately ensure the department-approved standard precaution training is complete.
