A 15 month old infant sleeping in the Infant "Lion Cubs" classroom, Room B119, in an unsafe sleep environment. Observed the infant sleeping on a crib mattress on the floor in the classroom. Correction: All infants must immediately be placed in a safe sleep environment. Immediately ensure all infants are provided with an approved sleep/rest equipment at all times. Notify Licensing Specialist in writing on how future compliance will be maintained. Correct by 02/22/2024
