2.223.C.1 · Each infant up to eighteen (18) months of age and enrolled in the infant program must be provided with an individual crib, futon approved for infants, or other approved sleep/rest equipment meeting Consumer Product Safety Commission (CPSC) standards published by the Consumer Product Safety Commission (CPSC) at 16 CFR section 1218.2 (April 23, 2015); 16 CFR section 1219.2 (October 28, 2019); 16 CFR section 1220.2 (June 3, 2023); 16 CFR section 1221.2 (January 20, 2020); 16 CFR section 1222.2 (August 5, 2023); 16 C.F.R. section 1236.2 (June 23, 2022); and 16 C.F.R 1241.2 (February 15, 2022) herein incorporated by reference. A 15 month old infant sleeping in the Infant "Lion Cubs" classroom, Room B119, in an unsafe sleep environment. Observed the infant sleeping on a crib mattress on the floor in the classroom. Correction: All infants must immediately be placed in a safe sleep environment. Immediately ensure all infants are provided with an approved sleep/rest equipment at all times. Notify Licensing Specialist in writing on how future compliance will be maintained. Correct by 02/22/2024

Identified that all three providers in the Infant "Lion Cubs" classroom completed infant safe sleep training, and a child in their care was found in an unsafe sleep environment. Observed P.O. completed safe sleep training in August 2023, and M.Z. and V.T. completed training in October 2023. Correction: Immediately ensure all staff at the center demonstrate knowledgeable decision-making, judgment and concern for the proper care and well-being of children. Immediately ensure that all safe sleep practices are followed, for all infants, at all times. Notify Licensing Specialist in writing on how future compliance will be maintained. Correct by 02/22/2024
