2.223.C.11 · Swaddling of infants must only be allowed with a health care plan completed and signed by the child’s health care provider.

Observed two (2) weighted Magic Merlin brand "Magic Sleep Suits" in two (2) infant cribs used by infants enrolled in the facility's infant program at the time of inspection; weighted Magic Merlin brand "Magic Sleep Suits" are not approved for use at child care facilities by the Department. Identified via staff interview and child file review both sleep suits are used by infants over the age of 12 months old. No evidence of Department-approved Health Care Plans on file for the use of weighted sleep suits and/or swaddling at the time of inspection. Correction: All infants must immediately be placed in a safe sleep environment. Immediately remove unapproved sleep

2.212.A · All staff at the center must demonstrate knowledgeable decision-making, judgment, and concern for the proper care and well-being of children.

Identified via staff file review that all staff employed at the facility have current, annual, Department-approved infant safe sleep training, and children in their care were identified as being put in an unsafe sleep environment at the time of inspection. Correction: Immediately ensure that all safe sleep practices are followed, for all infants, at all times. Correct Immediately - Respond to the Licensing Specialist with written confirmation of correction and plan for future compliance. Correct by 04/15/2026

2.230.C.3.a · Toddler climbing equipment must have a three (3) foot use zone surrounding the equipment. Toddler slides require a six (6) foot use zone extending out from the base of the slide.

Observed a two foot use zone extending out from base of Toddler slide on Toddler Playground. Correction: Toddler slides require six foot use zone extending out from the base of the slide - Correct Immediately - Respond to licensing specialist with written confirmation of correction.
