The outdoor, portable climbing equipment has two slides and a section for steps to climb up into the structure. The platform of this equipment measured approximately 24 inches in height. The placement of this portable climbing equipment does not allow for the required 6 foot use zones surrounding the two slides. A 4 month old infant placed in an unsafe sleep environment. Observed said infant was swaddled while sleeping in a pack n play in the main play room on the main floor of the home. Observed no evidence of a health care plan completed and signed by the child's physician stating child is allowed to be swaddled while sleeping. Observed several items (a few kids riding toys, small pieces of wooden stumps and an outdoor purple caterpillar kids tunnel ) within the use zones of this climbing equipment. Also, the placement of this portable climbing equipment does not allow for the required 6 foot use zone for slides. The use zone of blue slide FAX 303-866-4453 Phone 303-866-5948 ________________________________________________ measured approximately 3 feet, whereas the use zone between the green slide and orange slide measured approximately 52 inches. Correction: Immediately ensure there are proper use zones surrounding all climbing equipment at all times.
