A tricycle on the preschool playground to have a broken seat with jagged sharp edges creating a safety hazard to children. Fence on the toddler playground to have nails protuding along the wall and in the left corner creating a safety hazard to children. Sandbags used as weight for the shade umbrella on the toddler/infant playground to have a white plastic outer layer that is shredding creating a choking/safety hazard to children. Correction: All outdoor equipment and materials must be safe and free of hazards. Correct immediately. Director removed tricyle from the playground at time of visit. No response needed for that item. For all other items please submit verification of documentation and written compliance to specialist. Correct by 11/14/2025
