Identified through Assistant Principal Teresa's statement, that she is the newly assigned director for the ECE program and that she is functioning in a dual role. Identified through Teresa's statements that she is not present with the ECE program for 60% of any day that the center is open while she attendes to Assistant Principal duties. Reviewed Teresa's file and observed it to be missing documentation of director qualifications. This violation was previously cited on 3/13/23 and 4/5/23. This is a violation of stipulation E which states, "Licensee understands that the agency decision issued 7/26/19 regarding the waiver of the child care center Director requirement is rescinded. Licensee has 30 days from the commencement of the probationary license, to ensure that a qualified Director is present at the center at least 60% of any day that the center is open." Correction: The Director of the center must be present at the center at least 60% of any day that the center is open.

Prior to the original license being issued, following the renovation of the facility that would affect the licensing of the facility and at least every two (2) years thereafter, all child care facilities except family child care homes and Neighborhood Youth Organizations must be inspected and obtain an approving inspection report from the local department of health or the Colorado Department of Public Health and Environment and from the local fire department. These reports must be maintained at the facility and be available for review upon request by a Licensing Specialist.

Identified through Principal Jessica and Assistant Principal Teresa's statements that a current and approving fire inspection was not available for review by Specialists.
