Specialist asked staff member Maria in room H010IA how many children she had and Maria said 14 children, specialists counted 15 children and asked Maria to double check and Maria then confirmed 15 children. This is a direct violation of stipulation E of the probationary license which states, "At all times, staff members shall know the attendance of the classroom to which they are assigned. Facility Director shall be able to account for all children in care at the facility." 7.702.51.A.2.b · At the time of admission, the parent(s)/guardian(s) must provide for each child entering the center: Within thirty (30) calendar days of admission, and within thirty (30) calendar days following the expiration date of a previous health statement, the parent(s)/guardian(s) of each child must submit a statement of the child’s current health status or written verification of a scheduled appointment with a health care provider. The statement of the child’s current health status must be signed and dated by a health care provider who has seen the child within the last twelve (12) months, or within the last six (6) months for children less than two and one-half (2½) years of age. The statement must include when the next visit is required by the health care provider. All health statements must be kept at the center.

Exposed weed matting on the East Playground along the east fence in the woodchips accessible to children playing outside at time of inspection and posing a potential hazard. This is a violation of Stipulation O on the probationary license which states "Licensee shall ensure all indoor and outdoor play areas are maintained in safe condition and that hazardous, high-risk areas are made inaccessible to children." Correction: Immediately make all hazardous areas inaccessible to children. CORRECTED AT TIME OF INSPECTION - children playing on the East playground were moved to the grass and play structure to avoid areas with exposed weed matting.
