50 -54 Kindergarten through Second grade children were in cafeteria as one group with two staff members. Children were running and chasing each other around tables, standing on tables, rolling marbles across walking areas, throwing toys/objects at each other, hitting/scratching each other, and using toys as swords and stabbing each other in stomach lacking direct supervision and creating an unsafe environment for children. Sixteen third through fifth grade children in cafeteria with one staff member after switching areas with previous group. Children were running with scissors, went to the cement wall, and starting digging at the wall with their scissors creating a bigger porition of the wall to be damaged. They then ran over to pictures that were drawn by other children hanging on the wall and started to shred and tear several off the wall. Staff member was speaking to a couple other children and not directly supervising entire group creating an unsafe environment for children. Correction: Facility will submit a plan of action for the direct supervision of all children at all times. Correct immediately. Submit written response to specialist by overall due date.
