2.522.D.3 · The building based school-age child care center must provide access to an outdoor play area. The outdoor play area may be a city park or public school ground. The play area must meet the following requirements: The outdoor play area must be maintained in a safe condition by removing debris, dilapidated structures, and worn and broken play equipment. The center must identify hazardous, high-risk areas. These areas must be monitored to reduce the possibility of injury and accidents;

Observed ground cover to be worn away exposing sharp concrete footings around the base of a bench next to the climbing structure that may pose a hazard to children. Observed large portions of worn and peeling paint / finish on base of stairs. Correction: The outdoor play area must be maintained in a safe condition so as not to pose a safety hazard to children.
