During the walk through of the classrooms it was observed that classroom 145 had 20 children and is licensed for 19 children. Licensing specialist completed a walk through of the facility and all classrooms were outside on the playground and all 4 classes were lacking written attendance verification. Also in classroom 145 there were 10 children signed in and only 9 present. In classroom 137 the staff member stated they had 6 children and there were only 5 children signed in. Correction: Ensure that the times a child arrives and leaves the center must be recorded. Also staff members must complete written attendance verification through out the day including trabsistions. 7.702.72.C.8 · All outdoor play areas used for children’s activities must be checked daily and kept safe and free from hazardous materials or debris by removal of debris, dilapidated structures, and broken or worn play equipment. The staff must identify hazardous, high-risk areas; those areas must be made inaccessible to children to reduce the possibility of injuries and accidents.
