Licensing specialist observed a child leave the gym and went into the cafeteria without any staff noticing she had left. Child was in the cafeteria for roughly 10-15 seconds before they returned to the group on their own. Additionally, Dustin A., was supervising a child who was using the restroom when the doorbell rang. Dustin called for another staff to supervise the child while he answered the door but left prior to anyone coming out to replace him. The child was unsupervised for roughly 30-40 seconds before the staff member was able to break away from their group and come out to the hallway.
