A small carpet in the PS1 classroom was curled up on the edges and creating a trip hazard. A CD player was placed on a shelf by the toilet room in the PS1 classroom with the cord dangling and accessible to children. In the PS2 classroom there was a charger with the cord dangling and accessible to children. In the PS3 classroom cords for a CD player and mini speaker were dangling and accessible. Correction: Immediately (04/07/25) secure the carpet that is curling up on the edges and secure all cords that are dangling and accessible to children. Corrected during visit by staff member removing the small rug from the classroom and by staff securing all cords so that they are inaccessible to children. No written response is required.
