    Per discussion with Provider confirmed that CH2 and CH3 found a puddle in the backyard area behind the shed and were playing in the puddle. The area behind the shed cannot be directly supervised unless provider is standing right by it. Provider called children to come out after approximately 1 1/2 minutes. Correction: Immediately (06/28/2024) ensure that children are directly supervised around any standing water. Submit written verification of correction directly to Licensing no later than 07/05/2024. Correct by 06/28/2024 Stage II Licensing Concern : CH2 had played n the puddle in the backyard, then CH2's clothes were soaked, and CH2 was left in wet clothes for the rest of the day. - FOUNDED

    Discussion with provider confirmed that CH2 had played in a puddle and gotten their clothes wet. CH2 did not have any additional clothes and provider also did not have any clothes to change CH2 into. Provider stated POC of CH2 arrived to pick up CH2 within 15 minutes. Correction: Immediately (06/28/2024) ensure that children are changed out of wet clothing promptly. Submit written verification of correction to Licensing no later than 07/05/2024. Correct by 06/28/2024 ________________________________________________ Stage II Licensing Concern : CH2 alleged provider hit CH1 and CH2 on their hands. FOC of CH2 noticed bruises on CH1's side. Large bruises were alleged to be present on CH1's thigh. - UNFOUNDED Stage II Licensing Concern : Children are subjected to excessive time outs. The provider had their arm around CH1's neck when CH1 was sent to time out. - UNFOUNDED Stage II Licensing Concern : The provider was often on the phone or sleeping when children were in care. - UNFOUNDED Stage II Licensing Concern : Feces and food were present in CH2's underwear.
