One tube of toothpaste labeled as "Keep Out of Reach of Children" (KORC) was accessible in a container on the low counter in the bathroom area. Correction: Immediately (12/12/2024) Remove toothpaste from areas accessible to children. Corrected during visit by provider removing toothpaste from areas accessible to children.
