While reviewing medications and health plans that are managed and kept on-site it was identified that one prescription medication for a child in the program was kept in a locked medication bag. The licensing specialist requested to review the medication and healthplan kept in the bag. The program leader on-site at the time could not find the key to the medication bag and could not identifiy which child's medication was in the locked bag. Correction: All personnel and volunteers at the center must demonstrate knowledgeable decision-making, judgment, and concern for the proper care and well-being of children. Please ensure staff trainined and qualified to administer medication at the site have access and knowledge of the medications and health plans being managed by the program. Correct immediately and document compliance to the Department on the response form. Correct by 06/10/2026

The capacity and staff-to-child ratios required by regulation were not posted in the Gym and Cafeteria that are used as classrooms for the program.
