Observed children from Room 329 and Room 334 co-mingling on the playground at the same time. Identified there were 14 children in attendance in Room 329, and 11 children in attendance in Room 334. Identified the ages of the children ranged from 3 - 5 years old. This is a repeat violation from 4/29/25 Supervisory inspection. Correction: Immediately ensure to maintain appropriate group sizes, including during outside play on the playground. Resilient surface (wood chips) was not measured at the time of the inspection due to surfacing being frozen. Correction: Immediately ensure outdoor climbing equipment 18 inches or higher has resilient surfacing of at least six inches in the use zone surrounding the equipment at all times. Equipment must be inaccessible to children until staff can determine the surface is no longer frozen and surface measures at the proper required depth. Correct by 03/05/2026

2.230.C.6 · All pieces of playground equipment must be designed to guard against entrapment and strangulation. Any openings in gross motor equipment above ground must be smaller than three and one half (3 ½) inches or greater than nine (9) inches to prevent entrapment.
Measured the openings between the wooden slats and behind the steps of the loft in Room 332. Gaps measured between 7-8 inches wide. Correction: Immediately ensure the opening between the slats and steps of the loft are no more than 4 inches apart. Correct by 03/05/2026
