2.223.A.2 · The time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day, including during transitions.
Identified staff utilize "Brightwheel" to check children in and out daily. Identified some staff, but not all staff, are conducting name to face attendance tracking through "Brightwheel" when transitioning in and out of the classroom. Observed no evidence of Toddler 1 staff completing attendance verification when group transitioned to Toddler II for circle time today. This is a direct violation of stipulation I on the settlement agreement. Stipulation I states “Licensee will use a name-to-face attendance verification tool (paper or electronic form) and complete attendance verification for all transitions and at frequent intervals throughout the day. Licensee shall provide training for all staff on how to use the attendance verification tool and must maintain verification of the training in each employee's staff file". Correction: Immediately ensure all staff complete the attendance verification periodically throughout the day, including during transitions. Observed one "Melissa & Doug" plastic dramatic play baby doll, which was labeled for children ages 18+ months, on a wooden shelf accessible to children. Also observed a soft "Unipak" tree house with stuffed animals labeled "not recommended age under 3 years" in the Toddler II classroom. Correction: Immediately ensure all materials provided and accessible to children are appropriate for the ages of children in each classroom. Observed several packages of diaper wipes labeled "keep out of reach of children" in the Toddler I room an unlocked diaper changing table, accessible to children. 2.134.A · All child care providers must have a written plan for: evacuating and safely moving children to an alternate site; lockdown; shelter in place; and an active shooter on premises. The plan must include provisions for multiple types of hazards, such as floods, fires, tornadoes, and active shooter situations. The plan must be updated as changes occur and reviewed annually. All employees of a child care provider must also be annually trained on the provider’s written plan prior to caring for children, and retrained as changes occur.
