Care is provided in the basement of the Provider's home. Identified the window in one of the bedrooms in the basement is used as a secondary exit. The height of the window sill measured approximately 40 inches. Observed no evidence of a permanent access to this window at the time of the inspection. This is a repeat violation from 5/9/24 Supervisory Inspection. Correction: Immediately ensure there is a permanent access to these windows for the secondary exit. Permanent access can include a ladder bolted to the wall or furniture permanently attached to the wall. Notify Licensing Specialist in writing on how future compliance will be maintained. Correct by 04/09/2025
