2.329.H.2 · Soft bedding or materials that could pose a suffocation hazard are not permitted in cribs, futons approved for infants, or other sleep/rest equipment that meets the CPSC standards incorporated by referenced in rule 2.329.A. Soft bedding means, but is not limited to; any soft sleep surface like bumper pads, pillows, blankets, quilts, comforters, sleep positioning devices, sheepskins, blankets, flat sheets, cloth diapers, bibs, plush toys, pacifiers with stuffed animals attached, and stuffed animals.
A five (5) month old infant placed in an unsafe sleep environment. the infant was sleeping in a porta crib with a large bumper pad under the infants head, a child size sheet under the infant's body and another child size sheet covering the infant's body up to the infant’s neck, posing a potential risk of suffocation and Sudden Infant Death Syndrome (SIDS). Correction: All infants must immediately be placed in a safe sleep environment. Immediately remove the large bumper pad under the infants head, the child size sheet from under the infant's body and the child size sheet thats covering the infant's body. The Provider 1 corrected the violation at the time of the inspection by removing the large bumper pad from under the infants head, the child size sheet from under the infant's body and the child size sheet thats covering the infant's body. Identified that the Provider 1 Mais Haddad completed infant safe sleep training on 03/21/2025 and Provider 2 Wesan Sawaged completed infant safe sleep training on 03/17/2025 and a child in their care was found in an unsafe sleep environment. Correction: Immediately ensure that all safe sleep practices are followed, for all infants, at all times. Correct by 05/19/2025
