Asssitant Director indicated that both S1 and S2 received a final coaching/written for leaving the child unattended. The documents were signed by S1 and S2. The director will be having a staff meeting to discuss lack of supervision on 01/13/2023. LPAs obtained a copy of the coaching/write up which will suffice for the POC date of 01/09/2023.Director will submit a copy of the roster from the all staff by 01/14/2023 to LPA Zeron. Per Director, the subject staff member(s), no longer works at the facility effective 09/26/2022. Director stated, staff will begin to document children's napping time on diaper log. DIrector stated staff will begin to utilize timer on KinderCare tablet in order to keep track of diaper checks and changes. Based on records review and interview disclosures, staff member(s) did not change a child(ren)'s diaper within a two hour timeframe which left a child(ren) in a wet or soiled diaper for more than two hours which poses a potential health, safety, and personal rights risk to children in care.
