Interim Director provided LPA with a updated employee handbook policy that includes and enforces Hygiene and a Drug-Free Workplace. The new policy includes a Hygiene and a Drug-Free Workplace, Personal Hygiene, Prohibited Items, and Reporting Suspicious Activity. Staff reports and interviews indicating the subject teacher carried a strong odor of marijuana and had allegedly smoked on-site during break times. LPA collected a disciplinary write-up confirming the teacher was sent home and later terminated due to the incident.

Facility agrees to conduct a staff training with all infant teachers to review appropriate methods for calming and soothing infants when they are distressed at nap time. The training will be completed by July 17, 2025. This requirement was not met as evidenced by reports that the subject teacher yelled at the child and patted their back with excessive force, causing the child distress. Despite no visible injuries, staff intervention to modify the teacher’s behavior indicates the child’s comfort and well-being were compromised. The interim director agrees to submit proof of training completion by providing a training agenda along with a sign-in sheet that includes all current infant teachers, including the subject teacher. 101238(a) · Building Grounds. (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
During this inspection, Director made the drinking faucet of the sink inaccessible to children by placing an "Out of Service, Do Not Use for Drinking!" sign on the sink, along with placing tape around the drinking spout. Also, on October 15, 2022, the sink in the One Year Old classroom is scheduled to be replaced. Based on records review and observation, the sink in the One Year Old classroom has tested with a lead exceedance higher than 5 ppb. According to Director, the sink in the classroom is only used for handwashing, and water jugs and personalized water bottles are available in each classroom which poses a potential health, safety, and personal rights risk. During this inspection, LPA Hogue received a copy of service invoice for faucet/sink replacement. Director agrees to submit pictures, once the new faucet/sink is installed.
