The Owner/Director stated that S2 has already been terminated and held a staff meeting regarding supervision, with staff signatures documented and an email submitted to the LPA by the due date. Based on observations and interviews, on 3/18/2026, Child 1 (C1) was left alone without supervision at the sandbox on the playground for approximately three (3) minutes while the children were transitioning to the Orchard area outside of the facility. This poses immediate risk to the health and safety of the children in care. Director stated the facility ordered new sleeping mats. Director stated she will send a copy of the invoice/order form and photos of all new mats per classroom to LPA by due date of 6/24/24. Based on LPA observation napping equipment in room #1 (18 mats), #3 (21 mats), and #4 (13 mats) showed exposed foam. This poses as a potential risk to health and safety of children in care. Based on LPA observations, inspection Director stated the facility has stopped utilizing the orchard area. Director stated she will provide a document stating the facility will not or is not using the orchard until it is safe for children in care by due date of 6/24/24 LPA observed the pallets are not present at the orchard anymore. and interviews, there were wooden pallet in the orchard with nails sticking out of the pallet, and metal screws sticking out from metals attached to the wall. Wires were hanging from the wooden fence. This poses as a potential risk to health and safety of children in care. Director stated the facility plans to put a shade at the orchard. The facility will let the department know when they will start utilizing the orchard.

Director stated that the facility maintenance, Nitin Bhatia will fix the gates within 24 hours to ensure the gates swing away from the pool and the hot tub, and the gates are self-latching and self-closing. Director stated she will send a video proof of correction by due date of 4/4/24. Based on observations, the in-ground swimming pool gates do not self-close and do not self-latch and the hot tub gate do not swing away from the pool. This poses as an immediate risk to health and safety of children in care SUPERVISOR'S NAME: Martha Malane

A signed document about the training will be provided to LPA via email by due date of 3/28/24 Based on LPA observation, Room#4 had 13 children and only 1 staff. A signed document about the training will be provided to LPA via email by due date of 2/22/24 Based on LPA obsevation, Room#5 had 14 children and only 1 staff. Based on 2 staff interviews, another staff in the room was on break leaving 1 staff alone with 14 children. This poses as an immediate risk to health and safety of children in care
