Based on staff interview, Office Admin stated they will have a plan to prevent comingling from happening. Admin will train staff and review staff schedules and review parent contract 3 staff stated, there are days in the morning that infants and preschooler are together in one classroom until another staff comes in. Based on LPA's observations the infant classroom was out of ratio being 7 Admin Binbin Zhao stated they are recruiting 4 new staff in order to keep the facility in ratio. Admin Zhao stated if the facility is understaffed, they will call parents to drop off at a later time. A copy of this action plan will be sent to LPA by due date of 7/10/24. infants with 1 fully qualified teacher at 8:50am. Based on staff interview, S1 stated they are sometimes out of ratio in the infant room. This poses an immediate risk to the health, safety or personal rights to persons in care.

Office Admin Vicky Zhao stated the infant room is now fully staffed with 2 fully qulaified teachers and one aid that are scheduled full time. In case of a staff absence, there are at least two additional fully qulaified staff on site. Admin stated they will create an action plan to ensure the facility does not operate out of ratio by POC date. Based on LPA's interviews and record review the infant classroom has had many incidents of being out of ratio when all enrolled children attend and ongoing staffing issues. This poses an immediate risk to the health, safety or personal rights to persons in care.
