Facility will create the UIR's for the children that have received medical care for injuries at the facility and submit to CCLD by POC date. Facility will also conduct a staff meeting to discuss the reporting requirements. Based on interviews, it was revealed at least two children received injuries in the facility that required medical attention. Neither were reported to CCLD This poses a potential, health, safety or personal rights risk to children in care.
