The inspector came to the facility 3/25/25 but was unable to gather the samples to complete the Lead test and will return on April 8th. Licensee provided LPA with a receipt that shows Licensee ordered a canopy for the yard to use as shade for the preschool play ground. LPA observed a TB clearance for all staff. The deficiencies listed above have been cleared. During todays inspection LPA observed 2 of 8 staff have proof of immunization's. Per director, they have not Page 1.
