The center director stated that a staff training event was held at the center on 12/28/24 to cover the facilities care and supervision policies and provide training on active supervision. The director shall submit a copy of the meeting agenda along with the staff sign in sheet to the LPA by 02/12/25. Based on staff interviews conducted on 12/20/24 and 01/08/25 and the center director's own admission, it was corroborated that child C1-C3 were without staff supervision and visual observation for a period of time while in the center's outdoor play area. This posed a potential health and safety risk to the children in care.

101212(d)(1)(C) · Reporting Requirements. (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (1) Events reported shall include the following: (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child.
